Waiver & Consent to Receive a Tattoo

By booking an appointment via www.robtattooer.com or by signing this document in person, you confirm that you have read, understood, and agreed to the following:

​

1. Acknowledgement of Legal Age and Consent

​

• I confirm that I am 18 years of age or older, as required by UK law.

• I voluntarily give full and informed consent to receive a tattoo from Mr. A. R. Fawcett, or any authorised representative, acting on behalf of The Tattoo House RF Limited.

• I understand that I am entering into a contract with The Tattoo House RF Limited, and not with any individual artist personally.

2. Responsibility, Risk, and Limitation of Liability

​

• I understand that every reasonable precaution has been taken to perform the tattoo in a clean, sterile, and safe environment to the best of the tattooist’s professional ability.

• I acknowledge that once the tattoo procedure is complete and I leave the studio, all responsibility for the care, healing, and condition of the tattoo rests solely with me.

• I have been informed that aftercare guidance is available at www.robtattooer.com or can be provided in physical form upon request.

• I fully understand and accept that tattooing carries inherent risks, including but not limited to:

  ​

  • Allergic reactions

  • Infection (including septicaemia or sepsis)

  • Scarring or trauma to the skin

  • Swelling or bruising

  • Pigment rejection or under-skin pigment spread (blowout)

  • Cellulitis, adverse healing, or—in rare cases—loss of limb or death

  ​

• I confirm that I am not under the influence of alcohol or drugs and am of sound mind and judgment at the time of the procedure.

• I understand and agree that if any form of topical or local anaesthetic is used, whether disclosed or not, I waive any right to legal action or claim for damages against the artist or The Tattoo House RF Limited.

• I accept that The Tattoo House RF Limited and its staff accept no liability whatsoever for any complications, dissatisfaction, medical issues, healing outcomes, or future changes to the tattoo.​

3. Health Disclosure

​

I confirm that all information I provide is accurate to the best of my knowledge. I understand it is my sole responsibility to inform the artist of any medical conditions that may affect the tattoo procedure. This includes (but is not limited to) the following:

​

• Autoimmune disorders

• Heart conditions or pacemaker

• Blood disorders (e.g., sickle cell, haemophilia)

• HIV/AIDS or Hepatitis

• Diabetes

• Seizures, fainting, or epilepsy

• Eczema, dermatitis, acne, or open skin lesions

• Use of aspirin or other blood thinners

• Allergies to latex, Elastoplast, or topical lotions/creams

• Recent use of tanning beds or saunas

You must not be tattooed if you are currently pregnant or breastfeeding.

​

If my health status changes prior to or during the session, I will inform the tattooist immediately.

4. Agreement and Waiver of Liability

​

By signing this waiver—or by booking via www.robtattooer.com —I acknowledge the following:

​

• I have been given the opportunity to ask any questions and have received satisfactory answers.

• I fully understand and accept the permanent and personal nature of the tattoo.

• I accept that the care and result of my tattoo after leaving the studio is entirely my responsibility.

• I waive all rights to pursue legal action, compensation, or damages against Mr. A. R. Fawcett, any staff member, or The Tattoo House RF Limited in connection with the tattoo procedure, outcome, or aftercare.

5. Agreement to Terms

​

• I understand that all appointments booked via www.robtattooer.com are bound by this Waiver & Consent, even if a physical copy is not signed.

• I acknowledge that The Tattoo House RF Limited reserves the right to amend or modify these terms at any time, without prior notice.

• Continued use of the website, booking platform, or studio services constitutes acceptance of the most current version of these terms.

All information disclosed will remain strictly confidential.
